Item one: measured slab thickness, not builder promises
Every two-post manufacturer we sell specifies a minimum concrete thickness of four inches under the column pads, and we recommend six inches for anything you plan to use for tire and wheel work with lateral loading. The problem is that pole-barn slabs get poured by whoever is available that week, and the actual thickness often varies by two inches across a single pour. We drill a small pilot hole in the intended anchor location, feed a bent wire down, and measure. If it comes back under four inches, we either move the layout or the customer pours a new pad.
Our Nebraska farm-shop customer thought his slab was six inches because that is what the concrete guy quoted him. The actual measurement came back at three and three-quarter inches in the low corner. Item two: rebar or fiber, and how far apart

If you pour on May first, we do not drill anchors until May twenty-ninth, no exceptions, even if the surface feels rock hard at day ten.

Documentation is cheap. Do not skip it.
Item five: control-joint layout relative to columns
The saw-cut control joints in a garage slab are engineered failure planes. If a column base plate sits directly over or straddles a control joint, the concrete will crack along that joint first when loaded. We lay out every two-post install so that all four anchor bolts on each base plate land in the same slab panel, at least twelve inches from any control joint.
On the Nebraska shop the customer had cut a joint right down the middle of the intended lift bay. We shifted the layout eighteen inches, verified the ceiling clearance still worked with a laser, and drilled. That shift added no cost and it prevented a slow-motion failure that would have cost him a whole slab replacement someday. Item six: drainage slope and the anchor cone
Garage slabs are typically poured with a quarter-inch-per-foot slope toward a drain or door. That is fine for washing floors and terrible for a plumb lift column. The Nebraska shop had a half-inch slope across the lift footprint, which is on the heavy end but manageable.
