A good car lift annual inspection checklist is not a piece of paper you sign to keep an insurance adjuster happy — it is the thirty or forty minutes each year that keeps a two-ton pickup off your technician’s chest. We are Auto Lift Services, based in Ames, Iowa, and we spend a large share of every week under lifts in dealerships, fleet garages, tire shops, and farm equipment buildings from Council Bluffs to Dubuque. Over those years we have built a working checklist from what actually fails on Iowa equipment: rusted anchor bolts in salt-brine floors, frayed equalizer cables, leaking cylinder packings, and safety latches that were disabled years ago by someone long gone. Why an annual inspection is not optional

ANSI/ALI ALOIM is the standard that governs lift operation, inspection, and maintenance in North America, and it calls for a documented annual inspection by a qualified lift inspector. That language matters. “Qualified” does not mean the newest tech with a flashlight. It means someone who knows what a Rotary SPO12 carriage slide block is supposed to look like at 60 percent wear, and who can tell the difference between normal weep at a cylinder rod and a packing that is about to let go. OSHA does not publish a lift-specific rule, but it will cite a shop under the general duty clause when equipment fails and someone gets hurt, and the first document requested is the inspection record.

There is a business case too. Almost every insurance carrier we deal with in Iowa now asks whether shop lifts are inspected annually, and a few have started asking for the paperwork at renewal. We have also watched an annual inspection pay for itself in one visit — a fleet garage near Des Moines had an equalizer cable at roughly half its remaining strands, and catching that cost them a cable set instead of a dropped truck, a bent frame, and a workers’ comp claim. That is the honest reason we push a car lift annual inspection checklist on every customer, not the regulatory one.

Start with the paperwork and the nameplate

Before anything gets touched, we find the serial number plate and the capacity label. On a two-post that plate usually lives on a column near eye level; on four-posts it is often on the powerside column or the power unit bracket. If the plate is gone — painted over, sanded off, or replaced during a move — we note it, because a lift with no legible capacity rating is a lift nobody in the building can legitimately load. Replacement placards are available for most models and we source them as part of the visit.

Next we ask for the owner’s manual, the original installation record, and any prior inspection reports. In a surprising number of Iowa shops, none of the three exist, especially on equipment that came with the building. That is fine, but it changes the inspection. Without a manual we cannot confirm torque specs, factory shim configuration, or whether an aftermarket adapter set is even approved for the model. We also check the surrounding area: clear floor, no extension cords under the columns, unobstructed access to the shutoff, and legible operating instructions posted near the controls. A car lift annual inspection checklist that skips the documentation step tends to miss the biggest problem in the bay, which is that nobody knows what the equipment is rated to hold.

Anchors, concrete, and the floor underneath

This is where Iowa gets us. Chloride from winter brine tracks in on vehicles, drips off in the bay, and sits at the base of the columns. We pull the base covers and look at every anchor bolt for corrosion, thread damage, and whether the nut is even seated on a washer. Then we torque-check each one against the manufacturer spec — not tighter, because overtorquing a wedge anchor in tired concrete does more harm than leaving it slightly loose. Any anchor that will not hold spec gets flagged, and if a hole is spun out we relocate with an approved epoxy anchor rather than reusing the failed hole.

We also look at the concrete itself. Cracks radiating from the base plate, spalling, or a soft crumbly surface all suggest the slab is thinner or weaker than the lift needs. Most two-post lifts want a minimum of four inches of 3,000 PSI concrete, and plenty of older Iowa shop floors do not have it. We have cored floors that measured under three inches with rebar sitting an inch below the surface. When that shows up, the fix is a saw-cut pad pour, not a longer anchor. Column plumb comes next: we shim as needed so the columns are within spec, because an out-of-plumb column loads the carriage rollers unevenly and eats slide blocks fast.

The car lift annual inspection checklist for cables, chains, and sheaves

Equalizer cables and lifting chains are the components most likely to end an inspection with a red tag. We run a rag along the full length of each cable — a snag means a broken strand, and broken strands mean replacement, not adjustment. We look for kinks, birdcaging, corrosion at the sheave contact points, and flat spots where a cable has been running against a worn pulley. On chain-driven lifts we check for stretch, stiff links, and side wear on the plates, and we compare the two sides for uneven elongation, which points at a sheave bearing problem rather than a chain problem.

Sheaves and pulleys get spun by hand. Any grinding, wobble, or flat spot means the bearing is done, and a bad sheave will destroy a brand-new cable in a season. We check cable tension so both carriages arrive at the same height and the latches engage at the same tooth. If tension is far out of adjustment and has been for a while, we look harder at the slide blocks, because a lift that has been running crooked has usually chewed them. Everything in this section is in-stock inventory for the common Rotary, Challenger, BendPak, and Atlas models, so a failed cable rarely means weeks of downtime for a shop.

Safety latches, arms, adapters, and pads

We test every safety latch, at every height, on both columns or all four. The latch has to engage audibly, hold the load, and release only when commanded. Latches that have been wired back, pinned open, or filled with grease and dirt are common on older equipment, and that is an immediate stop-use finding. On four-post and inground lifts we check the mechanical locks and the lock cable or air release for the same behavior. A lift that only holds on hydraulic pressure is one seal away from being a hazard.

Arm restraints come next. Each arm has to lock against rotation when the lift is loaded and release cleanly when it is down. Worn restraint gears and stripped pins are routine on high-cycle two-posts in tire and brake shops. We inspect arm pins for elongation, check the arm pads for cracked rubber or missing hardware, and confirm that any stack adapters or truck adapters in use are approved for the model and rated capacity. Homemade blocks of wood or scrap steel under a frame rail get pulled out of the bay — we have seen a 4×4 split under load, and once is enough. Hydraulics, electrical, and the power unit

We pull the power unit cover and check fluid level and condition. Milky fluid means water intrusion, which in an unheated Iowa shop is common and leads to internal rust and pump wear. Dark, burnt-smelling fluid means the pump has been running hot, usually from low level or a plugged breather. We check every fitting and hose for weeping, look at hose routing for chafe points, and inspect cylinder rods for scoring or a steady drip past the packing. A slight film on a rod is normal; a puddle on the floor is not. Cylinder rebuild or replacement is a same-visit job on many models when we know the specs ahead of time.

Electrically, we verify the disconnect works, the motor is properly grounded, and the wiring is in conduit or approved flexible whip rather than zip-tied romex. We test the up control for dead-man operation — release the button and the lift must stop — and confirm the lowering valve does not creep. Then we run the lift through a full cycle unloaded, listening for the pump laboring, watching for jerky travel, and confirming both sides rise evenly. Finally we do a loaded test with a vehicle, checking drift over several minutes with the latches disengaged.

Documenting it, and what happens after

Every inspection ends with a written record: date, inspector, model, serial number, capacity, each item checked, findings, and a clear disposition — pass, pass with recommended repairs, or stop-use. We leave that with the shop and keep a copy, so next year’s visit starts with real history instead of memory. Deficiencies get sorted into two buckets. Stop-use items mean the lift comes out of service and gets tagged until the repair is complete. Recommended items are things trending toward failure — slide blocks at half wear, a hose with surface cracking, fluid due for a change — that we want documented so nobody is surprised in twelve months.

We also talk honestly about repair versus replacement. Sometimes a 25-year-old two-post with pitted columns, a spun anchor hole, and a discontinued carriage is not worth another cable set, and the money is better spent on new equipment with current parts support. Sometimes a lift that looks rough only needs cables, latches, and pads and will run another decade. Iowa shops get straight answers from us either way.
