A two post lift annual inspection is the single cheapest piece of insurance a shop can buy, and it’s the one thing we still see skipped year after year across Iowa. Most of what we find isn’t dramatic — a stretched cable, a worn lock latch, an arm restraint that stopped restraining. But those are exactly the parts that fail without warning. This article walks through what actually gets checked, who’s qualified to check it, and what it costs you to ignore it.

Why the Annual Inspection Exists in the First Place
The requirement traces back to two places: the Automotive Lift Institute’s ALI/SM standard, which lays out how vehicle lifts get inspected and maintained, and OSHA’s general duty clause plus 29 CFR 1910.184, which covers wire rope and slings. Neither one says “inspect it whenever you feel like it.” The industry standard is once every twelve months at minimum, performed by a qualified inspector, with written documentation kept on file. If a lift drops a truck and OSHA shows up, the first question is going to be about paperwork. The second is going to be about who did the work.
What makes a two post lift specifically worth the attention is the load path. On a four-post, the runways carry the vehicle and the cables are equalizers. On a two-post, everything — the entire weight of the vehicle — travels through the carriage, the arms, the arm pins, and either a chain or a hydraulic cylinder, with the equalizer cables keeping the two columns synchronized. There’s very little redundancy. A worn arm pin bore or a cracked weld at an arm knuckle is a genuine single point of failure. We’ve walked into central Iowa shops where the arm restraint teeth were completely rounded off and the tech had been “just being careful” for two years. That’s not a maintenance plan. That’s luck, and luck is a consumable.
Who Is Actually Qualified to Do It
This is where a lot of shops get tripped up. “Qualified” doesn’t mean the guy who’s been running the lift the longest. Under the ALI framework, a Certified Lift Inspector has been trained and tested on lift construction, inspection procedure, and the specific standards that apply. That’s a different thing than a manufacturer service tech, and both are different from a shop foreman with a flashlight. Some insurance carriers and most dealership programs now specifically require documentation from a certified inspector, not an in-house sign-off.
That doesn’t mean daily and monthly checks are worthless — they’re required too, and they’re your job. Every morning somebody should be looking at cables, lock engagement, hose condition, and the floor around the base plates. Every month you should be greasing what needs grease and checking bolt torque. But the annual is a full teardown-level look: covers off, carriage slides examined, cables measured for stretch and inspected strand by strand, chain elongation checked against spec, cylinder rods checked for pitting and seepage, anchor bolts torque-tested, and concrete inspected for cracks radiating from the anchors. If your inspector isn’t pulling covers, you’re paying for a walk-around. The Findings That Show Up Over and Over
After enough years of this, the failure list gets predictable. Equalizer cable damage is number one — broken strands at the sheave, corrosion where the cable sits in a wet spot, or simple stretch that’s got one column running an inch ahead of the other. Number two is lock latch wear. The locks are what hold the vehicle when hydraulic pressure bleeds off overnight, and worn latch teeth or a stretched lock release cable means the lift might catch on the next notch down instead of the one it’s supposed to.
Then come the arm restraints. On older Rotary SPO and Challenger units, the restraint gear and pin take a beating from techs dropping arms onto them, and once the teeth round over the arm can swing under load. We also find a lot of anchor bolt problems — bolts that were never torqued to spec at install, bolts sitting in concrete that’s too thin or too green, and spalled concrete around the base plates from years of freeze-thaw where a shop door lets weather in. Add in leaking cylinder seals, cracked overhead beam welds on clearfloor models, missing or illegible capacity labels, and dead-shorted limit switches that somebody bypassed with a jumper, and you’ve covered ninety percent of what a two post lift annual inspection turns up in the field.
What Happens When You Fail
Failing isn’t the disaster shops imagine. A failed inspection means the lift gets tagged out of service until the deficiency is corrected — and most deficiencies are parts, not scrap. Cables, lock assemblies, arm restraint kits, hydraulic hoses, seal kits, anchor bolts: all of it is available, most of it same-week for the major brands, and most repairs are a few hundred dollars in parts plus labor. Compare that to the cost of a dropped vehicle, a workers’ comp claim, and the OSHA citation that follows.
The genuinely bad outcome is a structural finding: cracked carriage welds, a bent column, or concrete that can’t hold an anchor. Those conversations usually end with a replacement recommendation, and we’d rather have that conversation with you than with your insurance adjuster. The good news is that a lift that’s had a real two post lift annual inspection every year almost never gets there. Problems get caught at the wear stage, not the failure stage. We’ve serviced twenty-five-year-old Rotary two-posts in Iowa shops that are still perfectly safe because somebody kept up with them, and we’ve condemned twelve-year-old lifts that nobody ever touched.
Documentation Is Half the Value
A verbal “yeah, it looks fine” is worth nothing to OSHA, your insurer, or a plaintiff’s attorney. The inspection record needs to identify the lift by make, model, serial number, and capacity, note the date and the inspector, list every item checked, document every deficiency found, and record what was corrected and when. Keep it with the lift’s manual and the original install documentation. If the lift has been relocated, keep the relocation and re-anchoring records too — that’s a question that comes up more than people expect.
We tell shops to keep a simple binder or a shared folder with a tab per lift. When a new tech starts, he can see the history. When you sell the building or the business, the equipment records add real value. And when something does go wrong, the difference between a documented program and a shrug is enormous in terms of liability exposure. Scheduling It So It Doesn’t Cost You a Bay
The pushback we hear most isn’t about money — it’s about downtime. Nobody wants a bay down on a Tuesday in October when you’ve got four alignments waiting. Fair. A single two-post inspection typically runs under an hour if the lift is in decent shape and clear of vehicles and clutter. Multiple lifts in the same shop go faster per unit because the tooling and paperwork are already out. Plan it for a slow morning, pull the cars out ahead of time, and have the manuals accessible.
We also strongly recommend bundling. If you’re already having somebody under the lift with the covers off, that’s the moment to change hydraulic fluid, replace a hose you’ve been eyeballing, swap a set of arm pads, and top off the grease. Combining the two post lift annual inspection with routine service means one visit, one setup, and one round of downtime instead of two or three across the year. Shops that schedule the same week every year — first week of January, say, or right after the summer rush — stop forgetting entirely. Making it a calendar event rather than a decision is the single most effective habit change we see.
